Patient Coordinator

Location: Leeds

Division: Retentions

Salary: £18,000 per annum + Bonus

Shift Pattern: 9am – 5pm / X2 11am – 7pm and X1 Saturday in 4

This role will be working within our Retention division

We are currently recruiting for a Patient Coordinator for our Retention division, within our Leads Office. This opportunity will be to help support the communication with our patients who are already booked onto our Diabetes Prevention Group programmes. This role focuses on helping patients to overcome potential barriers and ultimately lead healthier lives, longer.

This position will suit someone who has experience of working within a public health/promotion environment.

Duties:

· Contact existing patients who failed to attend their first group session and help understand how we can support them to attend their next session.

· Support patients who are in process of finishing their weekly group sessions and are about to start attending their new monthly sessions. This is to ensure a smooth transition experience for our patients.

· Building great working relationships with our patients by consistently delivering an outstanding level of care towards their journey on our programmes.

· Maintain accurate records of all Service User records and ensure that sensitive data is handled with the greatest attention to detail.

· Support the Service User engagement strategy to ensure adherence to the programme and minimise the drop out ratio.

· Complete office administrative tasks where required.

· Monitor the shared mailboxes and respond and action accordingly.

· Identify and escalate priority issues

  • You are not phased when it comes to working towards deadlines and can demonstrate great organisation skills.

Experience/Qualifications:

  • Experience of working within a Customer Care driven capacity – this is essential.
  • Experience of building strong working relationships within a customer orientated environment – this is essential.
  • Experience of working towards strict deadlines and KPI’s – this is essential.
  • Not only do you have telephone-based customer care experience, you also have experience of supporting queries via email.
  • Experience of working within a public health/promotion environment – this is highly desirable.
