Facilitation Officer

Location: Derbyshire 

Opportunity to Work Remotely – Laptop and Phone provide

Salary: £22,000 per annum

Contract Type: Fixed Term Contract

Working with local authorities, CCGs and employers, we design, implement and evaluate innovative health and wellbeing services that will result in long-term health benefits through sustained behaviour change.

The ICS Health & Wellbeing brand has been selected to deliver the NHS Diabetes Prevention
Programme (NHS DPP) across the UK. NHS England, Public Health England and Diabetes UK have committed to improving efforts to prevent diabetes. The programme is designed to deliver on Physical activity and dietary components, underpinned by behaviour change techniques.

We are currently seeking someone with strong Relationship management skills who can join us as a Facilitation Officer on a Full time, Fixed Term Contractual basis. You will be involved in the Coordination of the program across Derbyshire. 

Responsibilities

  • Engage and coordinate with GP Practices across Derbyshire.
  • Provide support in setting up the prevention programme templates on the GP Clinical System.
  • Provide advice and guidance as needed to support generation of referrals that come through.
  • Producing reports and reporting the progress of this to the Derbyshire Diabetes Prevention programme Group.
  • Work closely with Internal Stakeholders to identify problems and provide solutions for the Derbyshire programme. 

About You

  • Good communication and relationship management skills.
  • Strong Analytical skills and full proficiency of using Microsoft Office Packages.
  • Solid Planning and organisation skills for both internal and external planning.
  • A Problem solver and able to identify solutions within a timely manner.
  • Must have own car and full UK valid Drivers Licence.
