Regional Healthcare Case Manager

Location: Homebased with frequent travel

Hours: Full Time, Permanent

Salary: £30,000-£35,000 + bonus + Car Allowance 

Are you wanting to work within one of the UK’s leading Community Complex Care teams? Are you a people’s person seeking an opportunity to build new relationships? An exciting opportunity has arisen for a person centred, care focused individual to join our growing team of Case Managers. Utilising your relationship building skills, you will form an essential part of the Multi-Disciplinary Team. You will be responsible for providing person centred, bespoke care to all clients and their families. The Healthcare Case Manager’s role is to assist with service delivery so that individuals can live within their own home and communities, whatever their abilities.

Key tasks include:

  • Work collaboratively with the Clinical Team and Service Delivery Manager alike
  • To work with all internal departments and clients ensuring that the service we deliver matches what has been commissioned
  • To develop professional relationships with clients so that there is mutual trust and respect
  • Promote independence wherever possible, working with the clinical team to make sure social needs and goals are clinically safe
  • Support with conflict management within your caseload
  • Manage cases with a non-discriminatory, person centred approach, where the service user takes priority
  • Complete supervision and appraisals for workers within your caseload

Skills and experience:

  • Good level of basic education
  • Numerate & literate
  • Excellent communication skills
  • Excellent relationship building skills
  • Previous experience in the Healthcare sector
  • Insight and understanding of Complex Care preferred
  • Excellent problem solving/conflict management skills
